Personalized chronic disease guidance for patients and healthcare providers, instantly and at the point of care

Our Story

EAPOC (Evidence at the Point-of-Care) is a platform for computerized clinical decision support that was developed by a group of researchers at the University of Toronto in collaboration with patients, family doctors, specialists, educators, and pharmacists. In over a decade of work, each part of the system was carefully and systematically designed, built, and tested. Over 20 scientific articles related to platform have been published along the way, including clinical trials demonstrating that the system significantly improves patient care in real-world healthcare settings.

Our Technology

The patient or a care team member completes a quick online questionnaire. Our system then uses this information to determine the patient’s disease control level and any required guideline-based medication adjustments, and presents these to the provider in real-time. The EAPOC platform also creates a self-management solution called an action plan, which patients can access through a web portal anytime, anywhere. This empowers patients to self-manage their disease, improving their symptoms and helping them to avoid flare-ups and urgent healthcare visits. Finally, it also identifies patients who have severe disease and require specialist care for advanced therapies.
